Initial Visit

Please call our office 779-210-3724 to schedule a consultation or a treatment appointment, or visit the contact us page to email our office.

Please prepare for your visit by providing the following information at the time of your 1st appointment:

  1. State-Issued Photo ID
    Please bring a valid, state-issued photo ID with you to your appointment.

  2. Referral Slip and X-Ray Films (if applicable)
    Bring your referral slip and any x-ray films. If you have digital radiographs, they can be emailed directly to us. Please speak with our staff, and we will coordinate with your dentist to obtain them.

  3. Patient Forms
    Please arrive 15 minutes prior to your appointment in order to complete Patient Forms.

  4. Medical History
    Inform us of any allergies, medical conditions, and medications prior to your appointment. We may also need your physician’s name and phone number. If you have been advised to take antibiotics before dental procedures due to heart conditions, prosthetic joints, or other medical conditions, please follow your doctor’s recommendations.

  5. Dental Insurance Information
    If you have dental insurance, please let us know so we can provide an estimate of your insurance benefits for our procedures. Be sure to bring your insurance identification card with you, as this will assist us in processing insurance claims on your behalf.

  6. Anesthesia and Sedation Options
    Our goal is to provide high-quality dental care in a comfortable, relaxed environment. Local anesthesia (numbing) is used for all procedures to ensure they are pain-free. While this is sufficient for most patients, some may benefit from additional sedation. We offer Nitrous Oxide (laughing gas) and oral conscious sedation to enhance comfort. If you feel you would benefit from sedation, please discuss this with our scheduling staff for further instructions.

  7. Endodontic Consultation
    An endodontic consultation will be conducted before any treatment to confirm the need for treatment. If either you or your referring dentist have questions about the necessity of the procedure, a consultation can be scheduled as a separate appointment. Endodontic treatment is typically completed in one or two visits.
